Would you loose sleep over this? I plan on selling this truck when done so my name will be on it. Backside of the front fender. Looks horrible. Nothing is through and seems to pass Robert’s ice pick test. With everything else I have done to eradicate rust in this truck it is bothering me. To get it out and blast is a ton of work. If there was not this inspection hole I would be oblivious.

Personal opinion here. Scuff it up with a red scotch Brite and spray some epoxy over it. It's nothing that will ever be a problem. It's not in any elements and it's all barely on the surface.

I can’t get in there with my hand. It’s the backside I’m thinking more like cavity wax and make another access hole to reapply on occasion. Thinking this may me one those places @crashtech says it is better to just leave the rust alone.
 
This is a spot where I would use Phosphoric Acid. Put some in a small spray bottle, wear some gloves, and then cover the area. Repeat until it turns the rust black waiting the specified time between. Neutralize, rinse and then blow it dry with compressed air.
When dry check for any remaining white film/dust and clean with W&G if necessary. Let it dry and spray epoxy primer.

There are most likely drain holes somewhere in there so you might want to cover the floor underneath to protect it.
 
In the instance shown above I would get in there with a large orifice air nozzle to dislodge anything loose, then try to get some epoxy in there. It's going to have overspray on it anyway, might as well try to get a coat in there. Then cavity wax afterward as mentioned.
 
That probably doesn't look much different than the inside of the windshield post or the b-pillar on most of the cars you have restored, that will either ease your mind or put you over the edge.
