I have restored a number of vehicles over the years, and finding rust under filler on bare metal is common. The thing is, I have also found filler that was still in excellent shape with no rust underneath.
This leads me to believe that the rust under the filler is probably due more to bad metal prep prior to applying the filler. The other issue being the metal having holes in it that allows moisture to creep in from the backside. It was common practice back in the day, to put screw holes into the metal to help "hold" the filler.
Here's an example on the bed of this 1962 Chevy pickup:
You can see some of the rust I haven't removed yet along the top edge.
I ended up cutting that section of metal out and replacing it.
