Bumper prep fail

Two years ago I did an OEM bumper on a Honda CRV. Not exactly clear on my prep but was probably a grey pad to scuff. Don't remember if I used scuff paste. I did use epoxy sealer. That much I know.
Well, the primer has let go of the plastic. Paint is sticking to the primer beautifully. Do you think not scuffing with sandpaper could be the culprit? I really don't trust scuff pads.

Did you use the 700-1 and any Adhesion promoter?

Or do the Honda bumpers come with primer from the factory?

Now a days I always recommend using the 600-4 activated just in case.
 
If you bought it primed & primer let go, i say it isnt your doing.
My 93 Caddy bumper fillers, plastic trim i painted last year was actually shiney plastic. Couldnt believe it wasnt at least scuffed originally. I used heat gun, razors, tape to peel it all off. Used other brand adhesion promoter on front & i say made it brittle but still holding. Everything else i sanded & based right to it. So far all good knock on wood.
 
The OE primer came off the bumper? I've never seen that happen. It happens a lot on aftermarket covers, though.
